Billing and Insurance Information
Tennessee Spine and Joint Center participates with many insurance plans. Please contact our office to verify that we participate with your plan. Dr. Berklacich and Dr. Lowe do accept Medicare assignment. For your convenience we will file two insurance carriers for you. Although we will assist you in the processing of your claim, payment of the bill is ultimately your responsibility.
Co-pays are due at the time service is provided.
If your insurance company requires a referral from your primary care physician it is the patient’s responsibility to provide our office with the needed documentation. Please note that insurance companies will not back-date referrals. They are needed before the patient is seen by our office.
If you are covered by Worker’s Compensation it is necessary to alert the office so that your carrier can give prior authorization for treatment.
It is the patient’s responsibility to notify our office of any insurance changes. You will receive a bill from our office after your insurance company has processed your charges or if we haven’t heard from your insurance carrier after an extended period of time. All the non-covered services and remaining charges after your insurance company(s) has paid are due and payable within 30 days of billing. Please contact our billing office if payment arrangements need to be made. We accept checks, Visa and MasterCard.
